Component Membership Process

A general outline of the membership application process for each component society appears below. Applications should be completed in a legible fashion, and all required documentation should be included. Incomplete applications will delay processing. Please note that some components require you to attend a general membership meeting in order to complete membership application processing.

The majority of state component societies use the APMA membership application form, which can be found on the Join Us page (see exceptions below for Illinois, Ohio, and Puerto Rico). Questions regarding membership should be directed to the individual handling membership processing at the component. A current component contact list can be found on the State Components page. ALABAMA: Membership processing is handled by the AL secretary. The application is approved by the AL Board of Trustees. Processing is completed as soon as possible. Dues payments can be made to AL by check, MasterCard, VISA, Discover, or American Express, and payments are accepted on a semi-annual or annual basis.

ALASKA: Membership processing is handled by the AK president. The application requires a vote of the AK Board of Trustees. Processing is completed in approximately one month. Initial dues payments can be made to AK by check, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

ARIZONA: Membership processing is handled by the AZ secretary. Applications should be faxed to the secretary, who will notify the applicant of the dues owed. Upon receipt of dues by the treasurer and verification of the application by the secretary, the AZ Board of Directors will grant temporary membership. Permanent membership will be granted at a general membership meeting. Because general membership meetings are held semi-annually, processing can take up to six months. Initial dues payments are made to AZ by check, and payments are accepted on a semi-annual or annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

ARKANSAS: Membership processing is handled by the AR secretary/treasurer. The application process requires a vote of the general membership. Because general membership meetings are held annually, processing can take up to 12 months. Initial dues payments can be made to AR by check, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

CALIFORNIA: Membership processing is handled and approved by the CA staff and is completed as soon as possible. Dues payments can be made to CA by check, MasterCard, VISA, Discover, or American Express, and renewal payments are accepted on a monthly, semi-annual, or annual basis. Monthly payments are only available through automatic deductions from a checking account or credit card.

COLORADO: Membership processing is handled by CO executive director. The application is approved by the CO Board of Directors. Processing is completed as soon as possible. Initial dues payments can be made to CO by check, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

CONNECTICUT: Membership processing is handled and approved by the CT executive director and is completed as soon as possible. Initial dues payments can be made to CT by check, MasterCard, VISA, or American Express, and payments are accepted on a semi-annual or annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

DELAWARE: Membership processing is handled by the DE treasurer. The application is approved by the DE Executive Committee. Processing is completed as soon as possible. Initial dues payments can be made to DE by check, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

DISTRICT OF COLUMBIA: Membership processing is handled by the DC secretary/treasurer. The application is approved by the DC general membership. Processing is completed as soon as possible. Dues payments can be made to DC by check, and payments are accepted on an annual basis.

FEDERAL SERVICE: Membership processing is handled and approved by the FS executive director. The application process is completed as soon as possible. Initial and renewing dues payments can be made by check, MasterCard, VISA, Discover, or American Express, and payments are accepted on an annual basis.

FLORIDA: Membership processing is handled by the FL staff. The application is approved by the FL Executive Board (which provides provisional membership immediately) and then a vote of the general membership. Because general membership meetings are held in January (during the Science and Management Symposium [SAM] meeting), processing for full membership can take up to 12 months. Along with your application, an initial dues payment will be required based on your years in practice. If you have practiced one to four years, you must pay $250 initially, and if you have five-plus years in practice, you must pay $400. Renewing members can pay dues to FL by check, MasterCard, VISA, or American Express, and payments are accepted on an annual basis, or in three payments, or in four payments. State dues include registration for the SAM meeting.

GEORGIA: Membership processing is handled by the GA executive director. The application is approved by the GA Membership Committee and by the general membership. Processing is completed within six weeks. Dues payments can be made to GA by check, MasterCard, VISA, Discover, or American Express, and payments are accepted on a monthly or annual basis.

HAWAII: Membership processing is handled and approved by the HI treasurer. Processing is completed within one month. Initial dues payments can be made to HI by check, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

IDAHO: Membership processing is handled and approved by the ID executive director. Processing is completed as soon as possible. Initial dues payments can be made to ID by check, and payments are accepted on a semi-annual or annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

ILLINOIS: Illinois uses its own form, which can be found on the Illinois website. Membership processing is handled by the IL staff. The application is approved by the IL president, secretary, and executive director. Processing is completed within two weeks. Dues payments can be made to IL by check, MasterCard, VISA, Discover, or American Express, and payments are accepted on a quarterly, semi-annual, or annual basis. An additional fee applies if paying by the quarterly option.

INDIANA: Membership processing is handled by the IN membership services director. The application requires review by the IN Board of Trustees. Processing is completed within three months. Dues payments can be made to IN by check, MasterCard, VISA, or American Express, and payments are accepted on a quarterly, semi-annual, or annual basis.

IOWA: Membership processing is handled by the IA executive director. The applicant is initially granted pending membership until approved by the general membership at the annual business meeting. Because general membership meetings are held in the fall, processing for full membership can take up to 12 months. Dues payments can be made to IA by check, MasterCard, VISA, Discover, or American Express, and payments are accepted on a semi-annual or annual basis. IA includes registration for the annual fall CME conference in the membership dues. Additionally, upon written request, the IA Foundation will pay the state and national dues for first-year practitioners. (First-year practitioners are responsible for registration fees for the annual fall CME conference.)

KANSAS: Membership processing is handled by the KS secretary/treasurer. The application is approved by the KS Board of Directors. Processing is completed as soon as possible. Dues payments can be made to KS by check, and payments are accepted on a semi-annual or annual basis.

KENTUCKY: Membership processing is handled by the KY staff. The application is approved by the general membership. Because general membership meetings are held in the spring and fall, processing can take up to six months. Dues payments can be made to KY by check, MasterCard, VISA, Discover, or American Express, and payments are accepted on a quarterly basis.

LOUISIANA: Membership processing is handled and approved by the LA treasurer. Processing is completed as soon as possible. Initial dues payments can be made to LA by check, MasterCard, VISA, Discover, or American Express, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

MAINE: Membership processing is handled by the ME executive director. The application requires a vote of the general membership. Because general membership meetings are held three times a year, processing can take up to three months. Initial dues payments can be made to ME by check, and payments are accepted on a semi-annual or annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

MARYLAND: Membership processing is handled by the MD executive director. The application is approved by the MD Board of Directors. Processing can take up to four weeks. Dues payments can be made to MD by check, MasterCard, VISA, or American Express, and payments are accepted on a quarterly, semi-annual, or annual basis.

MASSACHUSETTS: Membership processing is handled by the MA staff. The application is approved by the MA Board of Trustees. Processing is completed within one month. Dues payments can be made to MA by check, MasterCard, VISA, or American Express, and payments are accepted on a quarterly, semi-annual or annual basis. There are additional fees if paying on a quarterly or semi-annual basis.

MICHIGAN: Membership processing is handled by the MI staff. The application is approved by the MI Board of Trustees and state divisional society. Processing is completed as soon as possible. Dues payments can be made to MI by check, MasterCard, VISA, or American Express, and payments are accepted on a quarterly, semi-annual, or annual basis.

MINNESOTA: Membership processing is handled by the MN executive director. The application is approved by the MN Board of Directors. Initial and renewing dues payments can be made to APMA by check, MasterCard, VISA, Discover, or American Express, and payments are accepted on an annual basis.

MISSISSIPPI: Membership processing is handled by the MS secretary. The application is approved by the MS Board of Directors. Initial dues payments can be made to MS by check, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

MISSOURI: Membership processing is handled by the MO staff. The application is approved by the MO executive director. Processing can take up to one month. Dues payments can be made to MO by check, MasterCard, or VISA, and payments are accepted on a quarterly, semi-annual, or annual basis.

MONTANA: Membership processing is handled and approved by the MT executive director. Processing is completed within three weeks. Initial dues payments can be made to MT by check, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

NEBRASKA: Membership processing is handled and approved by the NE treasurer. Processing is completed as soon as possible. Initial dues payments can be made to NE by check, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

NEVADA: Membership processing is handled by the NV secretary/treasurer. The application is approved by the NV Board of Trustees. Processing is completed within four weeks. Initial dues payments can be made to NV by check, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

NEW HAMPSHIRE: Membership processing is handled and approved by the NH secretary/treasurer. Processing is completed as soon as possible. Initial dues payments can be made to NH by check, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

NEW JERSEY: Membership processing is handled by the NJ staff. The application is approved by the NJ executive office staff. Processing is completed as soon as possible. Dues payments can be made to NJ by check, MasterCard, VISA, Discover, or American Express, and payments are accepted on a quarterly, semi-annual, or annual basis.

NEW MEXICO: Membership processing is handled by the NM staff. The application is approved by the NM Board of Trustees. Processing is completed as soon as possible. Initial dues payments can be made to NM by check, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

NEW YORK: Membership processing is handled by the NY staff. The application is approved by the NY Board of Trustees. Processing is completed within two months. Dues payments can be made to NY by check, MasterCard, or VISA, and payments are accepted on a monthly, quarterly, or annual basis. An additional fee applies if paying by the monthly option. State dues include registration for the NY Podiatric Clinical Conference.

NORTH CAROLINA: Membership processing is handled and approved by the executive director. Processing is completed within two months. Initial dues payments can be made to NC by check, MasterCard, VISA, Discover, or American Express, and payments are accepted on a semi-annual or annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

NORTH DAKOTA: Membership processing is handled and approved by the ND secretary. Processing is completed as soon as possible. Initial dues payments can be made to ND by check, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

OHIO: Ohio uses its own form, which can be found on the Ohio website. Membership processing is handled by the OH staff. The application is approved by the OH Board of Trustees. Processing is completed within one week. Dues payments can be made to OH by check, MasterCard, VISA, Discover, or American Express, and payments are accepted on a quarterly, semi-annual, or annual basis. Monthly payment plans can be set up by request and must be approved by the OH executive director.

OKLAHOMA: Membership processing is handled by the OK executive office. The application is approved by the OK Membership Committee. Processing is completed within 30 days. Initial dues payments can be made to OK by check, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

OREGON: Membership processing is handled by the OR executive director. The application is approved by the OR treasurer. Processing is completed as soon as possible. Initial dues payments can be made to OR by check, and payments are accepted on a semi-annual or annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

PENNSYLVANIA: Membership processing is handled by the PA staff. The application is approved by the PA executive director. Processing is completed as soon as possible. Initial dues payments can be made to PA by check, MasterCard, VISA, Discover, or American Express, and payments are accepted on a monthly, quarterly, semi-annual, or annual basis. Renewing members can pay dues through the Pennsylvania website.

PUERTO RICO: The membership form must be obtained directly from the Puerto Rico component society. Membership processing is handled and approved by the PR treasurer. Processing is completed within one month. Dues payments can be made to PR by check, and payments are accepted on an annual basis.

RHODE ISLAND: Membership processing is handled by the RI treasurer. The application is approved by the general membership. General membership meetings are held monthly except for the months of July and August. Processing is completed as soon as possible. Dues payments can be made to RI by check, and payments are accepted on an annual basis. State dues for members who attend the spring RIPMA meeting are waived.

SOUTH CAROLINA: Membership processing is handled and approved by the SC executive director. Processing is completed as soon as possible. Initial dues payments can be made to SC by check, MasterCard, or VISA, and payments are accepted on a semi-annual or annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

SOUTH DAKOTA: Membership processing is handled and approved by the SD secretary. Processing is completed as soon as possible. Initial dues payments can be made to SD by check, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

TENNESSEE: Membership processing is handled by the TN executive director. The application is approved by the TN Executive Committee. Processing is completed as soon as possible. Initial dues payments can be made to TN by check, MasterCard, VISA, Discover, or American Express, and payments are accepted on an annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express. The state dues include the registration fee for the state meeting.

TEXAS: Membership processing is handled by the TX staff. The application is approved by the TX Board of Trustees and then by the general membership. Because the general membership meeting is held in June, it can take up to one year for final processing. Applicants receive provisional approval almost immediately. Dues payments can be made to TX by check, MasterCard, VISA, Discover, or American Express, and payments are accepted on a monthly (in six installments with a 10-percent fee), semi-annual (with a 5-percent fee), or annual basis.

UTAH: Membership processing is handled by the UT secretary/treasurer. The application is approved by the UT Board of Trustees. Processing is completed within one month. Initial dues payments can be made to UT by check, and payments are accepted on a semi-annual or annual basis. Renewing members pay dues directly to APMA by check, MasterCard, VISA, Discover, or American Express.

VERMONT: Membership processing is handled by the VT secretary/treasurer. The application is approved by the general membership. Processing is completed within three months. VIRGINIA: Membership processing is handled and approved by the VA executive director. Processing is completed within two months. WASHINGTON STATE: Membership processing is handled by the WA executive director. The application is approved by the WA Board of Trustees. Processing is usually completed within two weeks. WISCONSIN: Membership processing is handled by the WI membership coordinator. The application is approved by the WI Board of Trustees and then by the general membership. Processing can take up to four months. Initial dues payments can be made to WI by check, MasterCard, VISA, Discover, or American Express, and payments are accepted on a semi-annual or annual basis.
