Faculty License Agreement
TERMS AND CONDITIONS FOR SPEAKER PARTICIPATION IN ANNUAL SCIENTIFIC MEETING
Thank you for agreeing to participate in the 2010 Annual Scientific Meeting, The National, in Seattle, Washington. In order to participate in the meeting you must agree to the following terms and conditions:
- I have read and agree to abide by the reimbursement conditions outlined in the APMA ANNUAL MEETING FACULTY EXPENSE REIMBURSEMENT POLICY including the section on accreditation that states: The APMA strictly adheres to the policies of the Council on Podiatric Medical Education. All Corporate sponsorship of educational activities for the Annual Scientific Meeting are made through an unrestricted educational grant paid directly to APMA. All contact with Corporate Sponsors shall be through APMA’s Development Department. Faculty of the APMA Annual Scientific Meeting may not solicit or accept any payment for their participation in the Annual Scientific Meeting from any corporation, organization or any other source, other than the APMA as specified in this policy. Any violation of this policy may be considered a breach in the APMA Code of Ethics and may be subject to appropriate penalties and sanctions.
- I agree to follow instructions with regards to booking travel and hotel arrangements and will meet the deadlines that are specified for these specific items.
- I agree to provide my power point presentation for review prior to the meeting on the deadline specified. I understand that I can make changes to my presentation after submission of my power point, but the general format and content of my presentation should be submitted by the deadline provided.
- I agree to follow instructions with regards to my presentation as to specific slides and formats and full disclosure of any potential conflicts of interest as required by the Council on Podiatric Medical Education (CPME).
- I agree to complete all required forms and agreements by the specified deadlines.
- I understand that failure to comply with the terms and conditions outlined above may jeopardize my being utilized as a speaker at this and future APMA meetings.
