As part of the growing OptumCare network, Reliant is at the forefront of value-based care with a business model of patient-centric and physician lead care teams focused on their patients’ total well-being rather than productivity.  * Provides comprehensive care to a select population of patients in the specialty  * Manage own patient panel in office * Assist each week in the OR No call * Would join a team of 5 APs, 8 Orthopods and 4 physiatrists * Performs age appropriate physical assessments, evaluation and treatment for primary care patient base seeking services from the organization * Takes history, including chief complaint, presenting illness, past medical history and interval history. * Orders lab studies when indicated. Identifies differential diagnosis. * Assesses severity of patient’s illness and establishes need/time for patient evaluation. * Discriminates between normal and abnormal findings in the history and physical in order to establish a diagnosis. * Performs tests and procedures as directed by diagnosis Establishes age appropriate plan of care related to patient’s identified problem, including medication, diet, therapeutic procedures, etc. * Consults with other health team members regarding proposed treatment guidelines as necessary
