The Centers for Medicare & Medicaid Services (CMS) recently published the proposed rule for the Calendar Year (CY) 2027 Medicare Physician Fee Schedule, which includes an arbitrary 50 percent payment reduction for services reported on the same day with Modifier 25. APMA strongly opposes this policy, which would unfairly penalize physicians for providing necessary, distinct care during a single patient visit and could create new barriers to efficient, timely care.
Over the coming days, APMA is taking that message directly to key decision-makers across the administration and Congress. APMA President Patrick A. DeHeer, DPM, will be in Washington, DC, this week for meetings on Capitol Hill and with White House staff. These meetings will give APMA a direct opportunity to demonstrate why the proposed reduction is misguided and why CMS should withdraw it before issuing the final CY 2027 Medicare Physician Fee Schedule. APMA is also scheduled to meet directly with CMS next week.
APMA is actively participating in the Same Day Care Coalition, a broad coalition of physician specialty societies and patient organizations working together to oppose the Modifier 25 proposal. Through the coalition, APMA is participating in meetings this week with Trump Administration officials and CMS to explain the real-world consequences this policy would have for physicians and their patients.
This is an all-hands advocacy effort. APMA is working with coalition partners, engaging administration officials, and meeting directly with congressional offices—but members of Congress and CMS also need to hear from you!
