A few years ago, CMS proposed creating separate E/M codes for podiatric physicians. APMA vigorously opposed this discriminatory proposal and preserved podiatrists’ access to the same E/M codes as other physicians. That effort ensured that podiatrists could benefit from subsequent improvements to those codes. Our analysis estimates that Medicare’s 2021 E/M changes—and the resulting increase in podiatrists’ use of higher-level E/M codes based on medical decision-making or time—have generated approximately $270 million in additional reimbursement opportunities for podiatrists since 2021. That’s approximately $4,000 per Medicare-billing podiatrist in 2024 alone.
Medicare contractors proposed restrictions that would have prevented podiatrists from being paid again when a patient legitimately needed another nail procedure within a certain period following an initial procedure. APMA fought those restrictions to protect physicians' ability to provide—and be paid for—medically necessary care. Estimates show this win protected up to $2,800 per podiatrist performing these procedures over a four-year period.
Thanks to APMA’s advocacy work alongside other physician stakeholders, Congress enacted a 2.5-percent Medicare physician payment increase for 2026. When combined with other changes in the 2026 Medicare Physician Fee Schedule, this change provided podiatrists a 4-percent overall increase in Medicare reimbursement. A 4-percent increase represents approximately $4,000 in additional Medicare revenue for every $100,000 in Part B payments to your practice.
The 2026 increase was temporary, but APMA’s work continues. APMA remains at the table with Congress and physician organizations advocating for permanent Medicare physician payment reform. We’re fighting proposed cuts for the 2027 fee schedule, and we’re aggressively opposing the catastrophic modifier 25 proposal that would slash reimbursement for needed same-day services.
